We are looking for an RN Clinical Coordinator for the Surgical Services Department at St. Joseph Hospital Eureka. Location: Eureka, CA Work Schedule: Full Time - 80 Biweekly Hours Shift: 8-hour, Days Job Summary: The Clinical Coordinator employs intellectual, interpersonal and technical skills through the utilization of the nursing process to assist patients in achieving maximum health potential. The Clinical Coordinator functions as a health team member through independent, interdependent and dependent roles within the health care system. Serving as the patient's advocate, the Clinical Coordinator assumes responsibility and accountability for nursing care for patients of various ages as defined by the department scope of service. Under the general supervision of the Department Director or Nurse Manager, the Clinical Coordinator supervises the nursing care for a given shift for a specific department. The Clinical Coordinator is responsible for ensuring the implementation of the medical plan of care, adherence to SJHS-HC policies and compliance to established standards of care/practice. Incorporates physical, motor/sensory adaptation, cognitive, psychosocial factors into the nursing process (e.g., behavior inconsistent with developmental stage, medications, educational needs, abuse/neglect, communication, etc.). Essential Functions: Assessment: - Delivers direct patient care in selected situations. - Conducts problem-oriented assessments according to standards for admission, ongoing re-assessment, transfer/discharge. - Correlates assessment data and interprets findings to identify individual patient needs and problems. Planning: - Initiates and updates patient plan of care based on patient condition, age, developmental status and progress, involving the patient, significant others and health team members. - Organizes interventions based on clinical priorities. Intervention: - Implements patient plan of care, including nursing, physician, and multidisciplinary directives, with compassion and concern for the patient's total need: emotional, spiritual, physical, comfort, safety. Advocates for each patient as a unique individual, deserving of compassion, respect and dignity without prejudice. - Assures that patient and family receive education to promote recovery and wellness during hospitalization and essential skills for safe self care after discharge. - Coordinates transfer and discharge with the healthcare team. Evaluation: - Effectively evaluates patient to recognize change in condition and intervenes appropriately (e.g., crisis intervention/therapeutic communication, and physiological changes). - Demonstrates competency in technical skills (e.g., psychomotor skills, equipment use and troubleshooting, IV therapy, medication administration). Operations: - Conducts problem oriented assessments of patient population needs and responds to the appropriate intervention. - Assures that patient care assignments are delegated based on patient acuity, staff skill level and scope of practice. - Acts as a clinical resource to staff, demonstrating effective problem-solving skills. - Effectively communicates significant findings and situations to the appropriate person. Keeps Nurse Director informed, in a timely manner, of any unusual concerns, situations, or conditions relative to staff, patients, families, physicians. - Provides nursing input to and solicits feedback from medical staff on a regular basis. Supervision: - Facilitates interdepartmental and intradepartmental problem solving and collaboration. - Serves as a positive, progressive, professional role model to nursing staff by contributing to effective management and operation of SJH. - Demonstrates knowledge and appropriate interpretation/communication or implementation of standards. - Identifies breaches in standards of care, practice, and conduct, and addresses them effectively with staff involved. - Provides coaching in a direct, supportive manner with feedback to the Nurse Director. Human Resources: - Supports budget parameters for shift staffing through monitoring and adjusting staff mix and number according to patient census and acuity. - Controls overtime. - Develops work schedules according to standards. Resource Utilization: - Ensures that appropriate supplies and equipment are available, accounted for and used in a cost-effective manner consistent with department budget. Safety - Helps to provide a clean, comfortable environment for customers and others by taking appropriate safety precautions and responds to urgent safety situations effectively. - Participates in monthly ERASR rounds. Productivity: - Organizes, prioritizes and completes activities and assignments in an efficient manner; supervises staff regarding same. - Demonstrates initiative in assisting others and performing other duties based on skill level and training orientation. - Delegates tasks based on the legal scope of practice and effectively supervises nursing care being given. Uses all work time productively in work-related activities. Quality Improvement: - Participates in department problem solving, suggests improvements, and facilitates change. CQI involvement on department/SJH level. Skills:

  • Ability to ensure patient confidence and comfort.
  • Efficiently communicates with physicians, customers and employees both in person and by telephone.
  • Reads and comprehends written instructions, able to interpret medical orders, correspondence, memorandums and medical records.
  • Applies deductive and common sense reasoning ability to problem solve on a frequent basis. Collaborates on issues relating to personnel and clinical matters. Minimum Position Requirements: Education: Coursework/Training - Graduate of an accredited nursing school. Licenses/Certifications:
    • State of California as a Registered Nurse.
    • Current CPR certification
    • ACLS Preferred Position Qualifications: Education: Bachelor's Degree in Nursing. Experience:
      • Acute care experience
      • Specialty experience.
