JOB SUMMARY: Under the supervision of the Center Manager, provides physical therapy services to Redwood Coast PACE participants according to departmental, professional and regulatory standards of care. ESSENTIAL DUTIES AND RESONSIBILITIES: 1. Provides initial assessment and at least quarterly reassessments of assigned participants to evaluate functional ability, identify major problems and set realistic treatment goals that may include ambulation, exercise and/or rehabilitation. Provides reassessment upon request of IDT, or upon change of condition. 2. Conducts reassessments to determine progress achieved and update goals Develops and implements a therapeutic Plan of Care with ongoing recommendations at least quarterly and when a participant's condition changes. Presents and coordinates the Plan of Care with the Interdisciplinary Team. 3. Implements an appropriate treatment program and monitors treatment progress. Supervises all aspects of participant physical therapy delivered by PACE Personal Care Attendants, therapy staff, and vendors as assigned. 4. Demonstrates proficiency in the set-up and application of all treatment modalities including exercise. Knows indications and contraindications. 5. Analyzes each assigned participant's needs for equipment, orthotics or prosthetics and assists in procurement as needed. 6. Develops and oversees a physical therapy maintenance program; trains Therapy staff and Personal Care Attendants in the Center to implement the program. 7. Monitors safe practices, is prepared in case of emergency and attends to good body mechanics. 8. Provides education to participants and family members or caregivers as ordered by the Primary Care Provider and according to physical therapy standards. 9. Participates on the Interdisciplinary Team, provides education to Interdisciplinary Team members and actively participates in Interdisciplinary Team meetings. 10. Works collaboratively with external physical therapists in the implementation of participant treatment and education for hospitalized/institutionalized participants. 11. Completes documentation in the medical record accurately and in a timely manner. 12. Participates with physical therapy program development and implementation as determined by Redwood Coast PACE needs. Physical Therapist, July 2022 Page 2 13. Maintains technical and clinical competency according to departmental, organizational and professional standards. 14. Understands fiscal issues and is appropriately involved in meeting budget targets. 15. Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others. 16. Consistently meets or exceeds Redwood Coast PACE targets for productivity. 17. Continually seeks better ways for delivering services and communicating with participants. 18. Consistently meets or exceeds Redwood Coast PACE customer service targets. 19. Demonstrates respect for and promotes participant rights including dignity, selfdetermination, access to care, confidentiality and independence. 20. Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to its communities. 21. Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success. 22. Complies with all policies and procedures of Redwood Coast PACE. 23. Demonstrates proficiency in delivering age-specific care. 24. Consistently meets or exceeds Redwood Coast PACE quality assessment and performance improvement targets. 25. Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area. 26. Acts in a manner consistent with agency values. 27. Other duties as assigned EDUCATION &/or EXPERIENCE: 1. Bachelor's Degree in physical therapy from an institution with a curriculum approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, the Council on Medical Education of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by the Centers for Medicare and Medicaid Services 2. At least one-year of experience working with a frail or elderly population required. LICENSES OR CERTIFICATES: 1. Must be licensed by the Physical Therapy Board of California. 2. Valid California Driver's License and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above. 3. Current CPR and First Aid certification within six (6) months of hire. PRE-SCREENING REQUIREMENTS: 1. Requires clearance of a DOJ and FBI criminal history background check. 2. Pre-employment physical, and PPD skin test. Physical Therapist, July 2022 Page 3 3. Motor Vehicle Report verification required. JOB SKILLS 1. Adapts appropriately to change of priorities and workload. 4. Able to maintain a consistent level of productivity. 5. Demonstrates sound verbal and written communication skills to convey information effectively. 6. Maintains a positive attitude and balance in relationships with others in complex interpersonal situations. 7. Incorporates an understanding of detailed requirements in work activities. Ability to collect and analyze program data. 8. Physical therapy knowledge and skills necessary to treat participants and manage complex problems. 9. Ability to provide care for the frail elderly. 10. Ability to work independently and within an Interdisciplinary Team. 11. Good organization and supervisory skills. 12. Ability to handle complex interpersonal situations. SUPERVISORY REQUIREMENTS: 1. This position has no direct supervisory requirements. PHYSICAL REQUIREMENTS: 1. Physical abilities sufficient to move between different staff work areas, communicate with coworkers and the public, operate a computer, produce reports, talk on the telephone, and travel to other agency worksites. 2. Ability to spend sufficient work time on feet, lifting and moving participants while using proper body mechanics and lifting techniques when assisting participants.
