*Job Description:*We are seeking a compassionate and experienced *Registered Nurse (RN)* to join our Home Health team serving Humboldt County, CA and nearby areas. The RN will be responsible for providing skilled nursing care to patients in their homes, ensuring high standards of care, patient satisfaction, and regulatory compliance.This role is ideal for nurses who thrive in an autonomous environment, enjoy a flexible schedule, and have a strong understanding of home health operations and documentation requirements.*Responsibilities:** Perform skilled nursing assessments and implement individualized care plans* Conduct patient visits (up to 6 per day) in alignment with productivity goals* Provide wound care, medication administration, disease management education, and other clinical interventions* Accurately complete and submit OASIS documentation* Collaborate with interdisciplinary teams, physicians, and family members to ensure continuity of care* Maintain professional and timely communication with the home health agency* Adhere to infection control and safety protocols* Travel within assigned territory using personal transportation*Requirements:** *Licensure:* Active RN license in CA (pending license accepted)* *Experience:* Minimum 1 year of recent skilled home health experience* *Documentation:* Proficient in OASIS (required)* *Certifications:* BCLS (Basic Life Support) required* *Schedule:* MondayFriday day shifts; one weekend shift required per contract* *Transportation:* Must have reliable personal vehicle for travel* *Other:** No first-time travelers accepted* No on-call duties* No floating required* Facility cannot accommodate block scheduling, 4x10s, or 3x12s* Will accept local candidates within a 50-mile radius at the same rate*Additional Notes:** The system is designed to group client visits efficiently, but occasional extended driving (50+ miles) may be required.* Candidates without OASIS experience will not be considered.* Time-off requests (RTO) must be confirmed with the hiring manager.*RN01*Job Types: Contract, TemporaryPay: $65.00 - $68.00 per hourExpected hours: 40 per weekMedical Specialty:* Home HealthPhysical Setting:* Nursing homeAbility to Commute:* Eureka, CA 95501 (Required)Ability to Relocate:* Eureka, CA 95501: Relocate before starting work (Required)Work Location: In personby Jobble
