City: Fortuna State: CA Start Date: Pending Privileges End Date: TBD Description: Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for. Includes some Chronic Pain Management Chart closure within 72 hours Schedule = 18 PPD, minimum see 18 PPD Monday-Friday 8-5 A mix of peds/adults (will consider FP only) Preference is they see infants but 12 required A decision is typically made within 7-14 days We will also accept DO BE will be considered if truly BE (5 years or less since residency) Active DEA is needed before the start Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely) The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30-day notice for ending all assignments. 1099 Pay Rate: Competitive Market Rates (W-2 Pay Packages available upon request) Certification Requirements: Please confirm credential requirements with VHS upon application.
