Location 2200 Tydd Street, Eureka, CA 95501 Requirements Candidate
CVs need to have the month & date on their work experience w/all
gaps in employment accounted for. Includes some Chronic Pain
Management Chart closure within 72 hours Schedule = 18 PPD, minimum
to see 18 PPD Monday-Friday 8-5 Must have FQHC experience Must have
experience and comfortable treating BH (anxiety) issues in peds
Must have experience with Pediatric Chronic Care Management A
decision is typically made within 7-14 days APC's with 5 years of
experience in a similar setting PAs must have a CA Controlled
Substance Education Course Certificate NPs must have a Furnishing
License with Schedules II Active DEA needed before the start
Credentialing takes 30-60 days (six weeks is standard for Locums
who submit app./documents timely) The following immunizations are
required for credentialing (before the start date): 2022-2023 Flu
Vaccine, TB Status/X-Ray Results (results must be within 90 days
before the start date), Positive MMR Titer or Complete Immunization
Series, Positive Hep B Titer or Complete Immunization Series,
Positive Varicella Zoster Titer or Complete Immunization Series,
Tdap Immunization (must be within the last 10 years), HCO & Vendor
agree to a 30-day notice for ending all assignments. Schedule
